QUESTIONS AND ANSWERS ON HIGH BLOOD PRESSURE WEIGHT AND TOBACCO
How important a factor is weight in blood pressure control?
Its importance would be difficult to exaggerate. Every extra pound of tissue, whether muscle or fat, needs an additional mile of blood vessels for its nourishment, and remember that the heart must pump blood through those additional blood vessels from 50 to 100 times a minute. Thus, the more we weigh the more work our hearts must perform.
However, not every fat person suffers from high blood pressure and not every thin person can boast of low blood pressure.
True. Still a clear-cut correlation exists. From ages 20 to 40, the rate of hypertension found among overweight individuals is double that found among those of normal weight. From 40 to 60, the HBP rate is 50 per cent greater among the overweight.
That’s all well and good but aren’t some people congenitally disposed toward being overweight? We all know people who eat a great deal and don’t gain weight and other people who eat less but who always seem to be fighting the “battle of the bulge”.
It is true that the metabolism of certain individuals allows them to burn up energy more easily than others. But the differences are frequently exaggerated. Overweight people usually eat more than others. And while fat people may have fat children, thus indicating a genetic disposition to obesity, they also, as Dr. Altschule has pointed out, tend to have fat pets. In other words, food habits not genetic inheritance generally determines whether we will weigh more than we should.
One inherent, inborn factor does play a significant role, however, and that is sex. Women tend to be naturally fatter than men and to have a harder time losing weight than do most men. Nature has given them an extra layer of fat to protect their unborn offspring once they become pregnant. This layer of fat, according to Dr. Barbara Edelstein of Mt. Sinai Hospital in New York, remains in place whether or not they ever become pregnant. From 15 to 30 per cent of the average adult woman’s body weight consists of fat. This compares to only 10 to 15 per cent for men.
This does not mean that a woman cannot and should not lose weight if her poundage exceeds the amount her doctor deems desirable, nor does it mean that her mate can ignore his own problems in that department. Most American men, like most American women, are overweight. What’s more, the situation concerning male obesity seems to be getting worse, not better.
Speaking of discomfort and Draconian measures, giving up cigarettes constitutes for some people the most Draconian measure of all. Yet virtually all doctors and other health practitioners believe that doing so will bring down blood pressure. Why?
Cigarettes tend to elevate blood pressure in many ways. The carbon monoxide in the inhaled smoke cuts down on the amount of oxygen which the heart would otherwise receive. This disrupts the heart’s normal efficiency and forces it to beat faster and harder to avoid oxygen starvation. Tobacco smoke also contains cadmium. Then, the nicotine in the smoke interferes with the electrical impulses which govern and guide the heart’s function. Tobacco smoke also constricts the arteries, forces the heart to pump harder. Finally, cigarette smoking is a highly addictive habit and therefore cannot help but cause increased stress. Anytime a person places himself in a situation where he needs something several times a day, and becomes increasingly tense when he doesn’t receive it, he is creating a lot of undue stress for himself.
