THE TREATMENT OF HIGH BLOOD PRESSURE
Some twenty years ago, when one of us was a junior doctor, a patient came into the clinic who was being treated with one of the first drugs for high blood pressure. The professor turned to his class and with the slight stammer that heralded one of his throwaway lines whispered, ‘For him to look so healthy he must have run out of his pills.’ As usual, the professor turned out to be right.
Today, fortunately, hypertension can almost always be treated without major side effects. There are three lines of treatment. The dietary approach is most effective for ‘mild’ high blood pressure. If the patient is obese, reducing his or her excess weight often lowers the blood pressure. (This is one of many reasons for trying to correct obesity.) Another treatment for mild hypertension is to limit consumption of salt to about 3g. per day.
By far the most widely used method is to treat high blood pressure with one or more drugs. One type has an action similar to the low-salt diet; these are the diuretics, which cause the kidney to excrete more salt. A second type, the beta-blocking drugs, decrease the volume of blood pumped by the heart, especially at times when this pumping action is enhanced (for example during exercise or anxiety); the beta-blockers may have other beneficial actions on the kidney. A third group acts on the blood-pressure-regulating areas in the brain; and still other drugs block the action of the sympathetic nerves, a powerful way of reducing blood pressure. So the modern treatment of hypertension has many strings to its bow, the outcome of outstanding research by academic pharmacologists, and later by the drug industry.
Today, surgical treatment of high blood pressure is used in rare situations. One is high blood pressure due to congenital narrowing of the great artery of the trunk, coarctation of the aorta. Another is hypertension caused by excessive hormone production by one of the adrenal glands, and a third is hypertension resulting from disease to one kidney; the surgeon can sometimes offer the patient permanent cure of high blood pressure.
