ANGINA AND EXERCISE: WHAT IS FITNESS?
Fitness is your functional capacity, that is your capacity to perform physical work. The amount of physical work you can do is dependent upon the amount of oxygen received and used by the body. The more it can use, the more work you can do. But, the more work you do, the more oxygen your body needs! The greatest amount of oxygen a person can use during physical work is termed maximal oxygen uptake. Physiologists define functional capacity in terms of oxygen consumption, so a person’s maximal functional capacity or fitness is measured by their maximal oxygen uptake. For example, the maximal oxygen uptake of a top athlete such as a marathon runner is roughly twice as high as that of an average person.
A person’s fitness or functional capacity is assessed by exercise tests on a treadmill or bicycle ergometer.
How is fitness affected by angina?
Cardio & BloodThe heart can increase the blood supply to the body by pumping more quickly, that is increasing the heart rate. Or it can increase the ’stroke volume’, that is, the amount of blood pumped at each beat. In order to do this, the heart itself needs more oxygen. The supply of oxygen to the heart is determined by the blood flow in the coronary arteries and the concentration of oxygen in the blood. At rest the heart extracts about 65 per cent of the available oxygen in the blood, so much of the higher oxygen demand during exercise must be met by increased blood flow. If the coronary artery blood supply is unable to meet the oxygen demands of the heart muscle angina occurs, since angina is caused by too little oxygen reaching the heart muscle.
Therefore, the fitness of a person with angina is limited by the blood supply to the heart muscle. This does not mean that people with angina cannot exercise or work. It does mean that there is a point at which the heart cannot increase its output, and to continue to demand that it does brings about the symptoms of angina. So, you can work and exercise just like anyone else, as long as you do so within your comfortable limit. In addition, by taking moderate regular exercise it is possible to improve fitness and extend that limit.
