RETURNING TO WORK WITHOUT HASSELS OR FEARS AFTER HEART ATTACK: MEDICAL AUTHORITIES DRAW RETURN-TO-WORK GUIDELINES
In 1989 a 70-member blue-ribbon panel of cardiologists, other health professionals, psychologists and sociologists, along with representatives from industry and insurance companies, drew up a set of guidelines regarding patients’ return to work following a cardiac event. The panel, headed by Dr Robert DeBusk, director of the cardiac rehabilitation program at Stanford University, published their recommendations in the Journal of the American College of Cardiology:
Most occupations don’t increase the risk of heart disease.
A clinical examination and testing to evaluate left ventricular function, heart muscle at risk and oxygen deficit to the heart muscle can be easily performed in most cases to provide an accurate prognosis. Cardiac catheterisation is not usually necessary.
Those whose occupations call for sudden or sustained high levels of physical effort, or exposure to temperature extremes, should be thoroughly evaluated.
Based on such evaluations, most patients can be advised to go straight to full-time work. But some may need a gradual comeback or a trial period.
Those whose work involves or potentially endangers the lives of others (such as air traffic controllers) should be carefully evaluated.
The patient’s functional capacity should be evaluated as soon as he or she becomes stable. That generally means five weeks after an uncomplicated heart attack, seven weeks following bypass, and one week after angioplasty. This will determine any abnormal responses to exercise. A full treadmill stress test at six months will be more definitive. Assuming that cardiac rehabilitation efforts have been faithfully followed, one can assume that the six-month stress test will show significant improvement and work capacity.
If routine testing procedures don’t give clear-cut information, more definitive measurements should be performed, including, perhaps, angiography.
Psychological profiles, including reactions to stress and Type-A behaviour patterns, should be taken.
If necessary, appropriate tranquillising medications can be prescribed. Others may respond well to psychological counselling.
Dr DeBusk, who is considered to be one of the world’s authorities on this aspect of cardiac rehabilitation, stressed the importance of the doctor-patient relationship. The doctor should spend significant and adequate time with the patient to discuss return to work, providing guidelines which should instil confidence and allay fears.
We’re going to talk more about the importance of that doctor-patient relationship, and how to make sure you get the best possible treatment and counselling, in the next chapter.
Not only should most heart patients return to work, but also they can and perhaps should do so sooner rather than later. For openers, surveys have shown that putting off a return to work lessens the odds that the patient will return at all. The quicker you get back to a routine lifestyleЂ”and that includes workЂ”the better your chances for complete recovery.
Dr DeBusk and his associates at Stanford studied the benefits of an occupational work evaluation in terms of shortening the time to return to work. About 200 patients recovering from an uncomplicated heart attack were assigned to one of two groups. In the first, they were given occupational work evaluation consisting of a symptom-limited treadmill test performed about 23 days after the heart attack and a formal recommendation to the patient and primary physician that the patient could return to work within the next two weeks. The other group received usual care, without the emphasis on work. There was no difference in age, medical status or occupation between the groups.
By the end of six months, 92 per cent of those getting the testing and encouragement had returned to work, while 88 per cent of those who did not get the additional attention also were back on the job. That’s not a big difference. But: return to full-time work occurred at an average of 51 days in those receiving special intervention, and 75 days in patients getting usual care. This 32 per cent cut in recovery time meant an average of $2102 in additional income! There were no more complications in the group that went back to work earlier. Couldn’t you use an extra two grand?

Take a copy of it to your doctor and ask that he or she fill in the appropriate blanks. Then you’ll “have it in writing” as to what your limitations and restrictions, if any, are, and you can proceed to live your life to its fullest.

Cardio & BloodIn the hours to days following a heart attack, there is a risk of serious heart rhythm disturbances (“electrical disturbances”), and severe weakness of the heart muscle as a pump. After initial recovery, a number of late problems can occur: the heart muscle can continue to be weak, and be unable to pump enough blood to meet the body’s needs. This can result in fatigue, weakness and shortness of breath from what is known as “heart failure,” which results in a higher pressure than normal in the lungs, causing shortness of breath. This shortness of breath most often occurs with activity, or at nighttime.
Sometimes the heart attack is incomplete, and chest pain or tightness can return, technically known as post-infarction angina. Heart rhythm disturbances can also occur, causing a rapid heart rhythm known as ventricular tachycardia, which is caused by a short circuit close to the scar of the heart attack. These short circuits, if they are present, can become activated for no obvious reason and cause a very rapid heartbeat which leads to dizziness, severe lightheadedness or loss of consciousness that can be life-threatening or even cause death, although fortunately this is not common. A less serious but troublesome heart rhythm disturbance is atrial fibrillation, an irregular heart rhythm that causes palpitations and dizziness but is rarely life threatening.

RECOGNITION OF HIGH BLOOD PRESSURE
It is almost never too late for treatment of hypertension to be worth while, but, as noted, the benefits are greatest if it is treated early. Hence it is important for the doctor to include a blood pressure check whenever he has the opportunity. If you have a family tendency to high blood pressure, stroke or heart attack you would be well advised to ask your doctor to measure your blood pressure, and to repeat the measurement every few years. This is the best use of a doctor’s time -prevention of later serious illness by a few well-spent minutes. Either way the result is reassuring for you: a normal result is comforting and so also is the knowledge that high blood pressure has been discovered and treated before harm has resulted.
Sometimes the doctor is tempted to dismiss a single high reading, to attribute it to anxiety in the hospital or surgery environment. This may be appropriate, but a single spike of high blood pressure can be the forerunner of persisting hypertension. As a minimum, the teenager or young adult with a single high reading deserves a check-up a year or two later; and it may be especially prudent for him to avoid obesity and, perhaps, to limit his intake of salty foods.
There is abundant evidence that high blood pressure (whether modest or gross in extent) increases the risk of heart attack and stroke. Other organs too are vulnerable to very high blood pressure. High blood pressure is easily diagnosed by the doctor, nurse or health visitor; and it is readily treated in the vast majority of patients. High blood pressure may also be to some extent preventable by avoidance of obesity, especially in adolescence and in early adult life. It is also possible that a low intake of salt will help to avert the condition.

This is not easy to do with others, such as children, in the house, but a compromise can be reached. To repeat, you need to develop pleasurable heart-healthy habits.

HEART ATTACK AND HEREDITY: WHAT IS IT THAT ONE INHERITS?
Is the proneness of some families due to the fact that relatives resemble each other in blood pressure, in cholesterol and triglyceride levels, in liability to diabetes and obesity, and even in smoking habits? As yet we have only limited answers. These family similarities are not very strong. Yet Professor Esko Nikkila’s research in Helsinki suggests that inheritance of similar blood-cholesterol levels and blood pressures largely explains why some families are prone to heart attack. This is a hopeful conclusion, for these disorders are readily treated. Professor Fred Epstein’s calculations suggest that other factors contribute to inherited risk too. Among these, lack of the protective lipoprotein HDL could be important; there is reason to believe that HDL levels are partly controlled by our genes. Very recently, Dr Feinleib has found that a tendency to low levels of HDL (often found in young men with coronary disease) may be inherited.
Whatever the inherited tendency is due to, it is chiefly operative in younger people. If a man is older than fifty-five years when he has a heart attack, his relatives have little or no excess risk; the same applies to women of sixty-five and over. As most heart attacks occur at more advanced ages, the impact of inherited risk on the population is small. In early-onset heart disease genes can play a more substantial role.
There are rare strongly inherited diseases which clearly predispose to heart attack. About one person in 300 has this disorder. Somewhat commoner is another inherited condition which causes high levels of cholesterol, triglyceride or both; this is called familial combined hyperlipidaemia. These conditions are inherited by about half of the members of affected families. To such families these disorders are certainly important: it is wise for all relatives to be screened and we believe that treatment should be offered to those found to be affected. They should in addition take particular care to guard against other risk factors. Familial hypercholesterolemia tends to increase the risk of heart attack, even in young people, if untreated.
But because these strongly inherited disorders are so uncommon, they make only a small contribution to the coronary disease problem as it affects the community. To reduce the risk of heart attack doctors must do more to detect people with these disorders, and so offer them the probable benefits of early treatment. But such conditions are uncommon and efforts at prevention must be made available to a far wider range of people.

ANGINA AND RELAXATION SKILLS: CUE-CONTROLLED RELAXATION
The aim of this method of relaxation is to pair the deep breathing with the progressive muscular relaxation so that the deep breath acts as a trigger or cue to relax. Before practising this method, it is best to become familiar with both deep breathing and progressive muscular relaxation first.
Once you are skilled at the progressive technique, you will be able to cue your body to relax by taking a deep breath and switching on the relaxation wherever you are. This will take time and effort to learn and will not happen at once.
Step 1. Begin by getting comfortable and preparing as though for the progressive muscular technique.
Step 2. Settle your mind, and bring into your conscious attention the relaxing scene you have been focusing on in the progressive technique. Whilst in this stage of relaxation, choose a word or phrase which seems to sum up the feelings you have of relaxation. For some people this may be ‘peace’ or ‘warm glow’ or ‘relax’, anything which can become your personal trigger.
Step 3. Begin by focusing on the lower half of your body just as you did for the progressive technique, but this time as you tense those muscles in turn begin the tension with a deep breath in. Remember to breathe from your abdomen and not from your chest only. As you get to the top of your breath you should be at the peak of the tension in the muscle group you are tensing. When you have reached this point begin the slow breath out, releasing that tension and mentally saying your cue word -’RE-LAX’ – or whatever you have chosen.
Cardio & BloodStep 4. Once you have worked through the major muscle groups, and this should take you about 20 minutes, you can enjoy the feeling of relaxation for a few minutes and then begin to come out of the relaxation. Follow the instructions of Step 4 of the progressive relaxation. You should finish feeling relaxed and calm, alert and invigorated.
It is important to practise cue-controlled relaxation regularly. About once to twice daily is ideal. Try to vary the times at which you do the practice and make a note of the effect that was achieved. This way you will begin to learn what time of day suits you best.

BEAT HEART DISEASE WITHOUT SURGERY: EXAMINING THE EFFECTS-CRITICS OF CHELATION AND WHAT THEY SAY-PART 1
Your doctor may have heard any one of the following things about chelation therapy:
it doesn’t work
it does work but it damages the kidneys
it chelates heavy metals from where they are stored in the body which then circulate through the body and cause damage
besides removing unwanted metals from the body it also removes the minerals which protect the immune system
in the period while it is removing the unwanted minerals it could deplete the immune system for long enough for cancer to occur
Let us take these criticisms seriously. Firstly, the flat statement that it doesn’t work.
It is a sad truth that the more general a criticism is, the harder it is to budge from the minds of those who perpetrate it. It is an equally sad truth that we have reached a point in organized health care where the opinions of the patient are not considered to be of value in proving or disproving whether a treatment works for them or not.
Levies of ‘empirical, subjective, anecdotary…the placebo effect’, are presented as if they render personal experience scientifically or morally wrong. The fact is that chelation therapy does work for the majority of people who have had it, as the meta analysis mentioned above has shown once and for all.
In preparing data for the meta analysis the researchers examined no less than 40 published reports about various effects of EDTA on circulation. Only 18 of them met their rigid scientific criteria and yet this still produced the result quoted above.
The researchers went on to say in their report that of the 40 studies examined, only one
multi-patient study conducted by Danish surgeons showed negative results. (Re-exarnination of this data has now indicated that their study was flawed – they used ineffective disodium EDTA instead of effective magnesium EDTA. Yet despite this, the British Heart Foundation still cites this one study as sufficient reason not to consider EDTA chelation therapy).
Quite apart from this extremely positive indication that chelation does work on circulatory problems, the general criticism that ‘it doesn’t work’ is completely inaccurate.
It has long been recognized by orthodox medicine that EDTA does work. It is a chelator of heavy metals, including lead, copper, mercury, cadmium, iron, calcium, etc, from the body, and is the standard medical treatment for those with excesses of these metals in their bodies. Organizations as august as the American FDA have accepted its safety for that purpose (proved in the AIMP trial).
Since the technique used to chelate the metals is exactly the same as that which is used for circulation therapy, what is the hue and cry about? In fact EDTA’s benefits to the circulation were first recognized as a result and by-product of its being used to treat patients with lead poisoning.
Cardio & Blood The allegations are derived from early experiences with chelating those with serious lead poisoning in which very heavy doses of EDTA were used very quickly – it had to be because the threat to life was considerable. As much as 10-20g of EDTA were used in those early chelations, which were often given daily for 14 days. Now the protocol allows for a maximum of 3g per infusion to be given slowly (over a 3-4-hour period) and that a minimum of 24 hours should elapse between each chelation.

ANGINA AND EXERCISE: POINTS TO REMEMBER ABOUT EXERCISE
There are three main points to remember about exercise and several minor ones. The main points are that it should be safe, it should be fun and it should be regular.
Exercise should be safe
You should learn how much you can do before symptoms arise and how to identify warning signals, e.g. tightness, shortness of breath, etc. So the way to keep exercise safe is to listen to your body. To ignore symptoms is highly dangerous and never try to ‘work off or ‘work through’ pain or discomfort.
The warning signals for a person with angina are pain, pressure or tightness in the chest, uncomfortable breathing or difficulty catching your breath, or any unusual sensations. These are a signal to stop exercising immediately. Of course, during exercise you expect to breathe a little more rapidly, your heart to pump more quickly and your muscles to ache to some extent, but it should never be excessive. Be guided by your knowledge of how your body usually responds to exercise and if in doubt about the messages your body is sending, you should stop exercising and discuss the matter with your doctor.
It is much better to plan your activity from the beginning so you can avoid getting into a situation where you have to carry on when you are already tired in order to get home. It is also helpful to plan to exercise to just below when you think you would get symptoms rather than to exercise until symptoms appear. If your body says that it has had enough, stop. Fitness can be achieved just as well by moderate effort as it can be by near-maximum effort – it will just take a little longer.
Pulse taking is often recommended as a method of monitoring exercise levels but if you want to take your pulse, which is a measure of your heart rate, use it as a guide to stop you from exercising anywhere near the rate at which you get symptoms. DO NOT use it to increase your activity to achieve a predetermined level. Sometimes the term age predicted maximum heart rate is used in conjunction with levels of exercise and pulse taking. The age predicted maximum heart rate is an estimate of the maximum number of heart beats your heart could manage in one minute for your age. The capacity of the heart to beat very quickly declines with age. You can work out your age predicted maximum heart rate using the following formula:
220 minus your age.
For example, if you are 53 years old:
220 – 53 – 167 beats per minute.
It is important to understand that this formula is an estimate of your maximum heart rate for your age and not an actual rate, which could be 20 beats above or below it. For this reason do not use it to set a target for exercise; rather, use it as a guide.
As a rough guide, even if you feel very good, do not exercise above ? of your age predicted maximum. If you are taking beta-blockers or other heart drugs which artificially reduce heart rate, you will not be able to use this formula, but you could still use the heart rate at which you get symptoms as a marker and exercise below it.
Some people are taught to take their pulse by feeling their carotid artery in the side of the neck but pressure on this artery can cause sudden slowing of the heart and a fall in blood pressure resulting in a blackout. Most people should be able to take their pulse in the radial artery in the wrist which is very safe. It is easy to learn to do. Place your fingertips lightly over the shallow dip on the front of the opposite wrist just below the mound of the thumb. Count each pulse over thirty seconds and double it to get the number of beats per minute.
The best form of exercise is the type which includes free rhythmical movement provided by large muscle groups in the body. Walking, cycling and swimming are all examples of this type of exercise known as dynamic or isotonic exercise. Taking regular dynamic exercise results in the benefits to the body discussed earlier. On the other hand, exercise where the muscles contract strongly but are prevented from moving, known as isometric or static exercise, is less suitable. Examples of isometric exercise are weightlifting and push-ups. This form of exercise can produce a sudden undesirable increase in blood pressure and workload on the heart. You should also avoid any home activity where you are pushing or pulling and therefore straining against a very heavy or immobile object such as pushing the car or shovelling snow.
Exercise should be fun
Exercise is of little or no use if it is regarded as something unpleasant or boring that you have to do. The exercise you choose should be comfortable and within your capabilities, interesting and most of all, enjoyable. There is such a variety of activities available now that there is something to suit everyone, however limited their exercise capacity may be. Many forms of exercise, for example, rambling, bowls and golf are also social activities providing the opportunity to meet people and make new friends as well as providing opportunities for physical exercise.
If possible, choose several varieties of exercise so that you do not become bored. Weather conditions may prevent you from pursuing certain activities sometimes, so a variety ensures that some form of exercise is always available whatever the weather.
Exercise is more enjoyable and there is less chance of injury if you are competent at the techniques of your chosen activity. It is not necessary to become an expert, but it is a good idea to do some background reading of books which are now available on even the most exotic of sports and or to get some practical help from more practised players.
Try to avoid competitive exercise as this adds a stressful element to the activity. It is sometimes difficult to stop exercising if it means letting down friends or team mates, and the pressures of competition may encourage you to continue exercising after you know you should stop.
Think about taking up an entirely new sport or activity that suits your interests and level of exertion. It could add a new dimension to your life.
Cardio & BloodIn order to gain the benefits that exercise can offer it must be performed regularly. It is important to remember this when choosing your activity, because those which require the hire of special equipment or which involve travel may not be practicable on a regular basis. Aim to do 20-30 minutes of exercise three times a week and gradually build up the intensity of the exercise. This will be sufficient to produce the adaptions of the training effect over a period of time between 6-12 weeks. Remember that fitness cannot be stored, it needs to be maintained by regular exercise, if you stop exercising for two or three weeks the training effect will gradually decline and fitness will be lost. For example, if you played sport at school, think how you ached after the first match of the term in comparison with the last match. That was because you had lost fitness before the first match during the holidays, but by the last match of the term you had played regularly and gained fitness.
There is absolutely no advantage in overexercising. It is only necessary to do just a little bit more than you would normally do to benefit. Indeed, the less fit you are when you start, the more you will improve and the more you will notice the benefits. It is a dangerous myth that exercise should hurt or ‘bum’. Generally, stiffness in the muscles should be avoided but if your muscles do become stiff after a period of exercise, allow the stiffness to go before the next session. Low-intensity exercise such as gentle walking may help to disperse the stiffness.

We now come to stress. One hears a great deal about stress these days. Does that mean that it is becoming an increasingly severe problem?

If the boss looked at you the wrong way, or if any member of your family showed signs of contracting a bad illness, you immediately might start to worry about the possibility of financial destitution.
Happily, such dire situations are, for most of us, things of the past. Less happily, however, the much more comfortable conditions we enjoy today create stress situations of their own. For example, today most people can afford to take vacations. But deciding just where to go, what to do and how much to spend can create all kinds of stress. The same holds true for so many other aspects of modern life. Most of us own many more clothes than our parents possessed. As a result, simply deciding what to wear each day has become, for some people, a stressful situation. In general, the more options one has, the more decisions one has to make, and decision-making can be quite a stressful experience.
How seriously does such stress affect blood pressure?
It would be difficult to overstate its effect. Stress stimulates the adrenal glands which prompt the heart to pump harder. The result is a rapid rise in blood pressure. De Paul University in Illinois checked the blood pressure of 254 students before and during a final exam. Before the exam, their average was 118/58. During the exam, it registered 152/113. This represents a nearly one-third increase in the systolic or top number and an almost 100 per cent increase in the diastolic or bottom number. It vividly indicates what a stressful situation can do to blood pressure. .
In another experiment, the blood pressure of a group of young adults was taken when they were calm and relaxed. Then each was individually told some bad news. The imparting of this information caused their blood pressure to rise were under 40 years old with a similarly aged group of patients who were suffering from other kinds of illness. He found that 91 per cent of the heart patients had undergone prolonged emotional strain associated with problems connected to their work. But in the other group of patients, only 20 per cent had experienced a similar level of stress. His findings led Dr. Russek to put emotional stress above diet, smoking, heredity, obesity and lack of physical activity as a cause of heart trouble.

The Calcium Factor-Calcium is one of the commonest minerals in the world. From earliest years we in the west are brought up with a large amount of it in our diets. Instead of beginning with the moderately calcified breast milk and being weaned onto a variety of solids, we are brought up on cow’s milk, which was designed for an animal with bones many times larger and denser than ours.
Dairy produce has become a western favourite and many ‘treat’ foods are composed around it – ice cream, cream itself, cream cheese, fromage frais (give a product a trendy name and sell it all over again), milk shakes, yoghurt drinks and yoghurt itself, cheese in its many forms, etc.
There is also abundant calcium in many of the popular foods we eat, besides it being in drinking water. But for now let’s assume that it is one mineral of which we are not in short supply.
Whether due to excesses in the diet or not, something seems to go wrong with the calcium metabolism around middle age. It is a metabolism which is constantly in and out of play in the body, since blood levels of it have to be kept within a critically narrow band, and to achieve this, thyroid and parathyroid hormones work in balanced opposition to each other, secreting hormones (calcitonin and parahormone) which stimulate the release of calcium from body stores, such as bones, or suppressing this activity by releasing hormones which stimulate calcium to be laid down in bone.
Calcium therefore moves from easily transportable states which dissolve in blood to
non-soluble states which can then be stored. It is this transportable ionic calcium which largely becomes part of arterial plaque and incidentally can become part of other calcified deposits in the body such as those around joints and old injuries. It is then known as metastatic calcium. (NB: ions are atoms or groups of atoms which carry an electrical charge, either positive or negative, of which more later.)
The minerals of the body work in harmony with each other but also in juxtaposition – so that one is balanced against another to achieve metabolic results. If the diet constantly provides excesses of one and deficiencies of another, then mineral imbalances occur and the body somehow has to cope with this by finding storage places for the excess and attempting to overcome problems caused by the deficiencies.
This is what is thought to happen when the body starts laying down calcium on artery walls. There is another hypothesis which suggests that this happens because the body ‘perceives’ that the arterial walls are becoming injured (by the action of destructive free radicals) and hence it deploys calcium as it does in other injury sites, to shore up the damage. Whatever the reason, the result is an accumulation of deposits which block circulation and prevent reabsorption of other plaque components that get shored up as well, such as fats, blood cells (especially platelets which are also used for repair purposes) and fibrous material, such as elastin and collagen.
Cardio & Blood Bruce Halstead writes in The Scientific Basis of EDTA Chelation Therapy that ‘there is an increasing amount of evidence that an abnormality in calcium metabolism is the biochemical lesion primarily responsible for heart failure’. He goes on to say that the lesion is believed to be caused by a difficulty arising in the exchange of the action potential involving calcium ions due to problems in the conducting cell membranes.
What this all amounts to in lay terms is that heart attacks can and do occur because of a faulty calcium metabolism on two levels: firstly, on the accumulative level of progressive calcification of arteries, including coronary arteries, and secondly, on the muscle spasm level due to the malfunction of heart cells themselves. Keeping in mind that mental and emotional input also affect this most sensitive organ and you may have the situation of a heart attack occurring where there is no significant blockage of the coronary arteries. All the more reason to get to grips with the causes, be they metabolic and therefore probably dietary or stress-related. But there are one or two other factors in this complex equation worth mentioning.

As stated before, cells produce the energy for life. If we feel tired, it is because we have dipped too deeply into our energy bank by overdoing things or because there is a malfunction in the energy production line. This is dangerous, because a body without energy reserves is a body without defence. A compromised immune system fails to destroy cancer cells, a job it normally does on a regular basis as they arise. A compromised immune system fails to conduct the body repairs needed to maintain integrity of its systems.
One of the ways in which a system can be compromised is by the destruction of its tissues by free radicals. These particularly affect the cell membrane – that is, the semi-permeable protective wall, which needs to perform well in order to let oxygen and nutrients in and carbon dioxide and toxins pass out. Cell walls are largely composed of fats (lipids) and, if these fats become oxidized, they change their quality as surely as culinary oils and fats do when they become rancid.
Heavy metal ions prompt this destructive process – especially iron (another excess in our diet caused by a high-protein intake far beyond a body’s needs, especially in later life when exercise/activity levels drop dramatically), but also copper and calcium.
This destruction of cell walls could be likened to a picketed factory where raw materials were not getting in or finished products out. Inside, at the assembly line, the mitochondria, or tiny energy producing units, attempt to make do with what is there. One of their main functions is to convert inorganic phosphate (ADP), sugar and oxygen into adenosine triphosphate (ATP). This energy-producing activity involves a chain of complex biochemical steps which are themselves dependent on an army of enzymes (organic catalysts) to make this burning process happen at body temperature.
If, for any reason, cell membranes are compromised, then ATP production falters and cells become energy-starved. This makes them more acidic which in turn attracts yet more ionic calcium into the cells. In the case of muscle cells (such as in the heart or in its coronary artery walls) this is a classic formula for them to go into spasm.
Cardio & Blood Calcium-channel blockers act to prevent absorption of inhibitory calcium ions into the cells, which is why they are so widely used today, but they do nothing to cure the underlying condition nor do they cure the energy deficit which also underlies it.
EDTA is actually capable of removing inhibitory ionic calcium from the cells, thus allowing the cells to resume energy production. Whilst removing excess calcium, it also removes other heavy metals which may also be interfering with cell activity such as iron, lead, cadmium, mercury, copper or even aluminium.

WHAT IS HEART DISEASE?
Heart disease is any problem or disorder of the heart, which is simply and basically a pump that receives “blue” blood, which has little oxygen, from the body, and whose right side pumps it to the lungs. “Red” blood, rich in oxygen, is then returned to the left side of the heart and pumped to the body by the main, strongest heart chamber, the left ventricle.
As is true of any other part of the body, many diseases can affect the heart. It has been claimed that almost a quarter of the 157,000 Britons who die of heart attack each year develop the disease chiefly because they are smokers. More than half of a smoker’s increased risk of dying is due to heart disease and stroke. People who have had a heart attack are much more likely to have a second one if they continue to smoke. And attacks of pain due to coronary disease (angina) occur more frequently in smokers.
Not only the coronary arteries but also those supplying the brain and lower limbs are affected by cigarette smoking. This can lead to one form of stroke. Narrowing of the arteries to the legs can result in gangrene, or may cause disabling attacks of pain in the legs on walking a short distance (intermittent claudication). In a hospital ward where patients with this condition are treated, one is likely to see a well-filled ashtray by every bed.
A few years ago a young woman came to the clinic with the complaint that she developed severe pain in both calves when she walked: she could manage no more than a hundred yards. She had impaired blood flow to the legs, confirmed by X-rays, and the diagnosis of intermittent claudication was made. It was due to atherosclerosis of the lower aorta. This is very uncommon in women of her age. She was found to have very high levels of blood cholesterol and triglyceride; this was corrected by weight reduction and a drug. She was advised to stop smoking, but could not give it up. Her circulation problem remained static for years. Finally she succeeded in giving up cigarettes. Within weeks of stopping smoking she became able to walk much more freely, having pain only when she hurried or climbed a steep hill. Her arterial disease was clearly related to her inherited high cholesterol state; but most of her improvement happened when she stopped smoking.
Another arterial disease which is many times commoner in smokers is aneurysm of the aorta. The main artery of the trunk, weakened by atherosclerosis, develops a ‘blow-out’ like a damaged tyre. If detected in time, it can be dealt with surgically; untreated it may lead to grave or fatal consequences.

ANGINA AND RELAXATION SKILLS: RAPID RELAXATION
The techniques of relaxation described so far have all required preparation and special conditions; a quiet, warm room, 20 minutes or so of time, and a private place. The advantage of rapid relaxation is that it does not require any of these preconditions. It will only take about two minutes to practise and can be done anywhere. However, it is not possible to bypass the other techniques, going straight to rapid relaxation and expect it to have enormous effects. You will only achieve the full benefit from rapid relaxation by first practising deep breathing, progressive muscular and cue-controlled relaxation. Rapid relaxation is a type of shorthand version of the full relaxation, but like shorthand you cannot use it unless you have learned to read and write first. Once you have learned this technique, however, you can use it any time and to the exclusion of the others if you so wish.
Step 1. Begin by reminding yourself of the word you use to cue the relaxation response. Stand up straight, with your feet about six inches apart and your arms hanging loosely by your sides. Close your eyes and focus your attention on your bodily sensations. Become aware of where your limbs are in space and feel the ground beneath your feet. In particular, become aware of your right arm, hand and shoulder, this is the one group of muscles into which you will put all of the tension in your body. Step 2. Begin by taking a slow deep breath, as before, and as you do so build the tension first in your fingers, clenching your fist, then in your forearm, elbow joint, biceps and shoulder. Keep your arm rigid, straight and close to your side. The peak of the tension should be reached when you have reached the top of your breath. Mentally say your cue word. Gradually let out the breath and the tension, pacing them together and matching the rhythm of the breath in. Imagine all of the tension dripping out of your hand and fingers as you stand relaxed and calm. It is very important to keep the rest of your body relaxed whilst you are focusing all of the tension in your right arm.
Cardio & BloodStep 4. Bring your focus back to your surroundings, take a deep breath in to invigorate you and shake your limbs loose for a few seconds.
This exercise has the great advantage of being usable whilst going about your normal activities. For example, while shopping you could stand and pretend to look in a shop window, or in an argument you could excuse yourself and go to the bathroom and return more in control, or immediately prior to exercise, say at the bottom of a hill. More about preparation for exercise at the end of the chapter.
