DIABETES IN PREGNANT WOMEN
Insulin
You will almost certainly have to change to twice-daily insulin injections if you do not already need them, and may need more frequent injections of short-acting insulin during the day. It is usual in the UK to transfer diabetic women taking oral hypoglycemic pills to insulin treatment before pregnancy, because of the potential risk of the pills damaging the developing baby. But do not panic if you find that you have become pregnant while taking oral hypoglycemic drugs. Available reports and animal studies show no definite risk of teratogenicity, or imperfect development of the baby. If you have become pregnant while taking the pills, just change to insulin as soon as you know you are pregnant. You will probably be able to go back to your hypoglycemic pills after delivery.
Some centers have used CSH pumps very successfully before and during pregnancy. Most rely on very close monitoring and frequent insulin injections.
Eat enough
Your baby needs energy to grow. This means that you must increase your intake of food as your pregnancy progresses. Beware the dangerous habit of missing food because your blood glucose is a little high. It is easy for ketones to form during pregnancy and they are not good for developing babies. To keep ketones at bay you need insulin and carbohydrate.
Insulin requirements
Theoretically, if you have some insulin reserve of your own, your insulin need may fall during the first ten weeks of pregnancy. In practice, this is not often obvious and what most pregnant women with diabetes notice is a gradual increase in the insulin need, which is greater than you would expect from your increase in carbohydrate. This is because your body becomes increasingly insulin resistant owing to the hormonal changes of pregnancy. By the end of the ninth month you may be on twice the dose of insulin you were taking when your pregnancy started.
