ALZHEIMER’S DISEASE: SLEEP
It is said that as we get older we need less sleep. This may be true for some old people but I am convinced there is a natural distribution with some needing less, a lot needing the same and some needing more (I certainly still need 8 hours minimum – prefer 10 and find 12 wonderful!) In Alzheimer’s disease the amount of sleep may not change, though many carers report that their relative appears to need a lot less sleep. What may happen however that is the sleep-wake cycle changes. It appears to reverse, meaning that people with Alzheimer’s disease tend to be awake at night and sleep more during the day. In itself this doesn’t matter except that where the person lives with carers or with other people nearby they can be very disruptive making noise, moving around and generally acting as if it were day. These disturbed nights can be the most exhausting aspect for carers.
As far as possible it is best to avoid the use of medication though it may become necessary. Keeping the sufferer as active as possible by day is often effective as they become marginally exhausted and sleep well at night. Many however are reluctant to be too active and also nap by day (giving the carer valuable time to do other chores). A small glass of brandy or sherry will do no harm and may lead to less disturbed nights, especially if associated with a warm drink (as long as incontinence is not a problem). Night sitting services are a boon to those lucky enough to be in an area where they are provided. The client is supervised and can be up as long as they want to, leaving the carer to get at least a few nights undisturbed sleep. It should be remembered that night cramps and painful joints may disturb sleep (as well as full bladders). For joint pains and other troublesome night aches two paracetamol can work wonders. Night leg cramps can be helped by drinking tonic water (due to its quinine content).
Inevitably however some medication is needed in order to give both parties some rest. The GP should always be consulted, but safe and effective sleeping medications include chloral hydrate/ Noctec (500 mg up to 1500 mg) and chlormethiazole/Heminevrin (one or two capsules at night). Sleeping medication is far more effective if used occasionally rather than regularly.
