CABBOHYDRATES AS NUTRIENTS FOR YOUR BODY: CLINICAL PROBLEMS
Nutritive adequacy
The Dietary Goals suggest that processed sugar intake be reduced to about 10 per cent of total calories. Why is this important? Perhaps the greatest single problem associated with a high sugar intake is that sugar contributes only carbohydrate and calories. Thus, with a high sugar intake it is necessary to get all the protein, minerals, and vitamins from other foods and still stay within the caloric requirement. By substituting complex carbohydrates for sugars, the mineral and vitamin content of the diet is improved.
Obesity
Sugar in coffee, honey on waffles, a candy bar, a soft drink sweetened with sugar, jam on bread, a piece of cake -l of these contribute carbohydrate and calories. It is very easy to eat sufficient amounts of these sugar-rich foods that the caloric intake exceeds the requirement. Thus, obesity results. For example, if you were to drink one 12-ounce can of a soft drink each day beyond your caloric requirement, the daily caloric excess would be 145 kcal. In one month the weight gain would be 0.56 kg (1.2 lb) or 3.7 kg (8 lb) in a year. Using sugar-rich foods in moderation, within one’s caloric requirement, remains a good rule.
Dental caries
Dentists usually warn their patients about the effects of sugars on tooth decay. Bacteria in the mouth digest sugars, leaving plaque and acids that lead to tooth decay. Two factors are especially important. (1) Chewy sweets stick to the teeth for long periods so that bacteria have ample time to grow and produce decay. On the other hand, a sweetened beverage leaves less sugar in contact with the teeth and the harmful effect is lower. (2) Frequent snacking with sweets provides many opportunities for bacterial growth and increases tooth decay. Two practical rules thus apply: reduce the number of times during the day when sweets are consumed; and brush teeth after each meal. If brushing is not practical, rinse the mouth vigorously with water after consuming sweets.
Lactose intolerance
Some ethnic groups (including blacks, Native Americans, Orientals, and others) produce insufficient lactase in the intestinal tract to digest lactose. As a result they have symptoms of flatulence, abdominal cramping, and diarrhea. Except in congenital lactase deficiency which is rare, the inability to digest lactose usually appears in late childhood or adulthood. Infants and young children of all population groups are almost always able to tolerate sufficient milk in their diets to meet nutritional needs. In some young children from 1/2 to 1 cup of milk may be tolerated at the time, while larger amounts produce symptoms of intolerance.
Hypoglycemia – current fad
Claims are often made that eating a high-carbohydrate diet is a cause of hypoglycemia (low blood sugar). However, hypoglycemia is seldom seen in people who eat diets much higher in carbohydrate than do most Americans. Hypoglycemia, in itself, is a symptom, not a disease. Its presence can be determined only by measuring the level of blood sugar under controlled conditions. There is no scientific evidence that symptoms such as fatigue, allergies, depression, abnormal child behavior, drug addiction, alcoholism, nervous breakdown, and others are caused by hypoglycemia.
Hypoglycemia does occur in certain clinical conditions such as (1) following removal of the stomach, (2) severe liver disease, (3) tumor of the pancreas, and (4) very occasionally in adolescents as a functional disorder. Because the treatment varies for each of these conditions, the cause of the hypoglycemia must first be determined.
Fiber
It can be said with some assurance that increasing the fiber content of the diet aids in normal bowel elimination. Many physicians have found that a high-fiber diet is helpful in preventing recurrences of diverticulitis. Much more research must be carried out before there is proof that a tow-fiber diet increases the risk of colon cancer, heart disease, and several gastrointestinal disorders. Also, there is no present proof that a high-fiber diet will reduce the risk of these diseases.
