MIGRAINE IN CHILDREN
Children may have migraine, and approximately one-fourth of our adult patients suffered from migraine during childhood. A major problem in assessing the incidence of migraine in children is that the symptoms are often different from those in the adult, with headache often being an inconspicuous part of the syndrome.
Although migraine headaches as severe as those in adults may be present in a child, very different symptoms may exist. The most striking features of childhood migraine are often episodes of abdominal pain, unexplained nausea and vomiting, listlessness, fever, and painful sensitivity to sunlight. Frequently these attacks precede or follow an exciting or tense event. In some children, stomach complaints are so severe and frequent that serious illnesses, such as appendicitis, are considered.
Childhood migraine may continue into adult forms or may terminate sometime in adolescence. It is interesting to note that the childhood form of migraine strikes both males and females equally, whereas in the adult population migraine occurs much more frequently in the female population. There are many possible explanations for this, but the exact reason why this change in incidence occurs is not fully understood.
The personality of children with migraine is often similar to that of an adult with migraine. These children are frequently intelligent, shy, and perfectionist. Not uncommonly, their parents are educated and successful and often impose, quite unintentionally, unreasonable pressures for achievement on their offspring, who would ordinarily perform well in school anyway. These children are easily frustrated, tense, and high strung. In some families, children tend to be discouraged from any showing of anger, even when it may be appropriate.
Pain Relief/Muscle Relaxant It is very important to recognize that when children are in pain, parents may show more love, responsibilities may be lessened, and expectations for performance may be reduced. These create a certain advantage to having pain, a phenomenon generally termed secondary gain. It is our opinion that it is vitally important that physicians or other health professionals treating young people in pain counsel not only the patient, but also the family.
If a child begins to appreciate too frequently the “power” of illness, it may well influence his/her personality, character, and relationships with other people. Once this happens and pain becomes a component of the way an individual relates to his/her world, influencing relationships, educational efforts, and other important decisions, then successful treatment of this problem will require much more than simple treatment for headaches.
